A crew, working for Red Diamond Energy Services, was laying down 2 7/8-inch tubing. The employee on the floor would unlatch the elevators slightly above the floor. The joint would bounce and the employee on the ground would grab and slide the tubing down the rack. While the 29th joint was being laid down, the employee's right ring finger was crushed between the tubing and the floor as the joint bounced resulting in amputation.

The injured employee was overseeing that the rod connections were going well. A work crew was coming out of the well with 1.5-inch, fiberglass rods and double stands. They had just made the connection. An approximately 60-foot fiberglass rod unscrewed from the holding, slipped out of the transfer, fell out of the barrack, and hit the rig floor. It then bounced off the rig floor onto the ground and struck the injured employee on the left side of the head, knocking him unconscious.

An employee was pushing a stand of drill pipe to its stowed position. While pushing the drill pipe, he slipped and fell forward. He placed his right hand on the deck to catch his fall. The stand of drill pipe swung back towards the injured employee as the driller was lowering the top drive to catch the stand, before it swung across the mast. The stand of drill pipe was then set down on the injured employee's right hand, amputating his ring finger.

An employee on a work over rig went to the rig platform to relieve the rig operator. Wind was blowing very hard (30 to 40 mph) with gusts. An umbrella was attached to a handrail to protect employees from elements and blowing dust. The employee was adjusting the umbrella when a strong gust blew him off of the rig platform. The gust carried the umbrella, the 4-5 foot handrail, and the employee over the edge. He fell approximately 15 feet to the ground, breaking his femur.

An employee was working on a centrifuge on top of a centrifuge stand. The employee lost footing while stepping down from the centrifuge and fell backward into the handrails.
